What are the office hours?

Monday – Friday 8:00am to 5:00pm (clinic hours vary by Provider)

Do you accept walk-ins?

Walk-ins are not accepted; however same day appointments are available by calling the clinic appointment desk at 8:00am.

How do I schedule an appointment?

Call the Health Services Department clinic appointment desk at 915-858-1076.

What do I need to take to my appointment?
  • Insurance card
  • PRC card
  • Current medications you are taking
  • Discharge paperwork for Emergency Room follow ups
  • If referral is being requested, any supporting documents pertaining to referral
What do I do if I need to cancel or reschedule my appointment?

Call the clinic appointment desk at 915-858-1076 to cancel and reschedule your appointment. Appointments must be canceled 24 hours before the scheduled time. Cancelations less than 24 hours will be considered a no show.

What do I do if I miss my appointment?

Call the clinic appointment desk at 915-858-1076 to reschedule the missed appointment.

Are there any fees for canceling, rescheduling, or missing an appointment?

There are no fees charged to the patient, however there is a no-show policy that came into effect 01/01/18. After a patient has 3 no-shows, patient will be counseled, after 5 no-shows the patient will not be allowed to schedule an appointment with any of the Health Services clinics.

 


 

Purchased Referred Care (PRC)

 

How often do I need to update my PRC card?

Once a year on your birthday. However, you have PRC coverage 7 days after your birthday.

What do I need when I update my PRC card?
  • Proof of address/utility bill
  • Driver’s license/state ID
  • Proof of insurance
  • Completed ACA forms if uninsured
  • Rent receipt with your name on it. If the bill is under someone else’s name, bring a signed letter with the utility bill.
