Community Health Center

Purpose - The primary purpose of the Ysleta del Sur Pueblo Community Health Center (CHC) is to assure that all tribal members have the opportunity to be healthy in mind, body and spirit. Much of the CHC's work focuses on improving the health of  pueblo community through education.  The CHC also adresses individual health care needs.

 Mission Statement -The Community Health Center (CHC) provides health resources that enhance the wellbeing of the tribal community thus, provides a means for a better quality of life.

 

Organization - The Health Department is organized into several service divisions: Contract Health Services, Diabetes Program, Podiatry Program, Community Health Representative, Optometry, Dental Services, Medical Records and Patient Registration, and the Prevention Program.

  • Diabetes Program -The diabetes program is a case management and early detection program that monitors diabetic patients, handles patient follow-up, provides referral assistance and conducts early detection screenings for undiagnosed individuals.   •  Community Health Representatives CHR - The Community Health Representative (CHR) offers tribal members a health care support system, health care assistance and links to medical services and/or medical information.
  • Podiatry Program - The Podiatry Program's main target is diabetic tribal members, but all members are eligible for the podiatry program.  The podiatrist plays an important role in the prevention and management of foot complications (i.e., poor circulation, ulcers, and nerve damage) due to diabetes.
  • Contract Health Services (CHS) - The purpose of the Ysleta del Sur Pueblo Contract Health Services (CHS) program is to assist eligible tribal members with essential medical services not available at CHC.  Among other duties, CHS negotiates and implements medical service agreements with local healthcare providers and also processes payment claims.  Services are dependent on the availability of funds.

  • Optometry Program - The optometry program offers professional eye care services to all enrolled tribal members.  The Optometry Program provides exceptional vision care in a friendly, caring and competent manner.  The staff strives to attain all patient vision goals.  This program provides vision exams and glasses to tribal members at no-cost.  An optometrist is available at the community health center twice a week. 
  • Dental Care – Enrolled tribal members are eligible for preventive and comprehensive dental care.  Services consist of preventive treatment such as cleanings and sealant placement.  The dentist also conducts routine exams and procedures such as x-rays and fillings.  If specialty care such as oral surgery or complicated extractions are necessary, the patient may be referred to a specialist.
  • Medical Records and Patient Registration - The Medical Records and Patient Registration Department maintains the patient database system.  This component of the Community Health Center (CHC) system registers tribal patients; updates records as necessary; and inputs important data such as insurance and medical history.  In 2005, CHC handled over 10,000 encounters inclusive of:  telephone consultations, client appointments and enrollment updates.
  •  Prevention Program - The Prevention Program is a relatively new program.  It addresses the surge of preventable diseases afflicting tribal members such as heart disease and diabetes.  To tackle unhealthy lifestyles that contribute to disease, CHC has initiated prevention measures that include screening all tribal members at least once a year through education, vital lab screening and if necessary appropriate health referrals.
